About

The session will provide attendees with information on Adult Family Care and Personal Care Management programs.  Northeast Arc programs will share information on both programs and eligibility/requirements.  Following the presentation, registrants will receive the recording, presentation slides, and a resource guide with additional information on the two types of programs and providing agencies in the Northeast Region.

The Northeast Transition Collaborative Network (NTCN) is an initiative from Northeast Arc’s Community Resource Center.  NTCN connects families, educators, and providers in the Massachusetts Northeast Region to support youth transitioning from school to adult services. Our goal is to unify resources, offer trainings, and create a collaborative space to promote person-centered planning and successful outcomes.

Professional Development Points are available for Educators in attendance of the live webinar!

The Northeast Transition Collaborative Network (NTCN) is an Approved DESE Professional Development Provider in Massachusetts.
Please note that Professional Development Points (PDPs) can only be awarded when the following requirements are met:
  1. Minimum of 10 hours of professional development completed.
  2. Assessment of learning, which may include an end-of-course assessment or an observable demonstration of learning.
  3. Evidence of mastery, showing that the participant has met the provider’s criteria.
To meet these requirements, NTCN will send formative and summative assessment surveys at 3 months and 6 months after completion of the professional development. These assessments will:
  • Confirm total hours completed.
  • Collect written reflection on what was learned and how it has been applied.
  • Serve as evidence that the participant meets our mastery criteria.
Certificates:
  • After each webinar or in-person session, educators seeking PDPs will receive a Certificate of Attendance for their records.
  • An Official PDP Certificate will only be issued once an educator completes 10+ hours of approved professional development and the required assessments.
